WHY WE COLLECT INFORMATION ABOUT YOU

sk:n hold clinical information about you to ensure practitioners have a complete and continuous record about your past, current and future treatment. Your records may be written down, or held on a computer.

To identify who you are we need your:

  • Name

  • Address

  • Date of birth

In some cases we may require your NHS number.

Information is held solely for the purpose of your health and wellbeing and will only be shared with practitioners and agencies involved in your treatment and care.

If you have agreed we will use your data to send you email and text messages for marketing and promotional reasons.

HOW WE KEEP YOUR INFORMATION CONFIDENTIAL

  • All sk:n staff members have a duty of confidentiality and will treat your data with dignity.

  • We store your files appropriately and securely, only allowing the relevant members of staff to access them.

  • The organisation actively implements and regularly audits security measures, to ensure your information is safe.

HOW TO ACCESS YOUR RECORDS

We have a strict Subject Access Request policy and your information will only be provided to you when the sk:n Information Governance team are satisfied of your identity. 

You can request access to your information in writing or you can ask to see your records whilst in a consultation with your practitioner, they will then explain what they have written.

When a formal request has been received in writing, the relevant person will: 

  • Forward the written request to the sk:n Information Governance Team

  • Forward proof of your identity to the team, i.e. a utility bill (within the last three months) and a photographic ID

  • By Law, sk:n have a month in which to provide you with a copy of your records.

WHAT ELSE CAN YOUR INFORMATION BE USED FOR?

  1. Your information can be used for clinical audit following or during a consultation or treatment
    Clinical Audits review current standards of patient care against acceptable best practice. Audit results are discussed by practitioners to make improvements to patient care. Audit results can be published and/or presented, but always in an anonymised format so that individuals cannot be identified. 

  2. Your information could be used for research
    Research seeks to investigate new treatments, interventions and management procedures so that treatment outcomes are continually improved. Clinical Audits monitor what has been done. Researchers look at what could be done in the future. 

  3. Your information may be used to help manage and plan services
    It may be used in an anonymised format for measurement such as waiting times, quality of information and care, and information given about treatment. 

  4. Your information could be used to educate our staff involved in your treatment plans and care.
